The 29-year-old man was on the C/ General Ricardos in Madrid at around 10.30 when he suffered a severe blood-sugar drop, or hypoglycaemic episode, causing him to pass out.
His car veered onto the pavement and ploughed into four women in their 60s.
One, aged 62, broke both her legs whilst another of the same age and a woman of 63 suffered multiple cuts and bruises, and a fourth, said to be 58, may have broken a bone in her foot.
None of them sustained serious injuries, although the woman with the broken legs may be in plaster for several weeks.
Luckily, an ambulance was passing at the time on the way from another call-out, meaning paramedics were able to attend to the women immediately.
They also treated the driver as a matter of urgency and rushed him to the Jiménez Díaz Foundation hospital.
His condition is believed to be at least moderately serious, but full details have not been revealed.
